The position: Case manager for UnitedHealthcare
Job duties: Responsible for overseeing medical management activities across the continuum of care. Makes calls to assess members' current health status. Identifies gaps or barriers in treatment plans. Provides health education and treatment decision support. Interacts with medical directors on challenging cases. Encourages members to make healthy lifestyle changes. Coordinates care for members and makes referrals to outside sources as needed.

Requirements: Requires a current, unrestricted RN license in the home state. Applicants must have at least two years experience in a hospital setting or experience as a telephonic case manager for an insurance company. United prefers candidates who are certified case managers, have a BSN or have experience with discharge planning. Experience in utilization review, concurrent review or risk management is preferred.
